Spacer in q220 regulator?

Found out why my adapter hose didn't seem to want to work. Anyone know why this was on the "needle" part of my valve where the bottle would connect? A 1 lb bottle worked fine but this spacer made the adapter hose for a 20 lb tank not work. Pulled these out and it works fine with no leaks

I wonder if that piece came from some adapter.

That spacer and O ring are part of the adapter hose end. Wonder if they came off from the previous owner, maybe he had a faulty one?
