Need Help ASAP (need judging scorecard)

I am having a family rib cookoff tommorrow and wanted to know if anybody has any scoring sheets that our judges could use to rate the ribs.
 
Each entry will be scored by 6 FBA certified judges in the areas of PRESENTATION, TASTE / FLAVOR, and TENDERNESS / TEXTURE. Scoring ranges from a low of 5.0 to a high of 10.0 in increments of .5 points for each category.
