Silly question, but what is the proper way to setup a heater meter alarm. It never seems to work for me. I set the setpoint for my meat probe and put a positive value in the threshold, I never get the alarm. I have applied the settings. Test sends me a message no problem

If it sends the message it seems the alarm is triggering properly, do you see the popup on the web interface when the alarm triggers? Could be an issue with your buzzer circuit, if you have the latest firmware you should hear a chirp when you power up the device, do you hear that.
There is just one BS170 and one resistor in the buzzer circuit, pretty easy to troubleshoot

Threshold is the trigger value, not "Setpoint". Setpoint is an action that occurs (set the setpoint when the alarm is triggered). It sounds like you're putting your trigger threshold in the setpoint field.
